NOTE: Dogs from OUTSIDE OF WISCONSIN: *The exhibitorās name and address *Identification of each animal exhibited, including number of animals, type and description. (Breed, age and sex would be an adequate description.) *Certificate of Veterinary Inspection obtained from a licensed veterinarian stating the age of the dog, and if over 5 months, the date of the last rabies vaccination and the revaccination due date.
